Cigarette ash probably caused the fast-moving fire that killed a police captain and most of his family in Carmel, N.Y., authorities said.

The lone survivor of the fire, the captain's son, flicked the cigarette ash, which ignited a bed of mulch in front of the house, police said.

Larchmont police Capt. Thomas Sullivan Sr., his wife, Donna, and their two teenage daughters died. Thomas Sullivan Jr., 20, survived.
